About The Role

Eminence Home Care CT is seeking a skilled and compassionate Licensed Practical Nurse to join our team. As a Per-Diem LPN, you will play a pivotal role in providing quality healthcare and compassionate support to our patients in the comfort of their homes. At Eminence Home Care CT, we believe that our patients deserve the best care possible, and we strive to provide them with the quality of care that they deserve.

Responsibilities

Administering medication as prescribed by the patient's physician in accordance with state regulations
Performing basic patient assessments, including vital signs and monitoring health status changes
Providing wound care and dressing changes, as needed
Assisting patients with daily living activities, such as bathing, dressing, and grooming
Educating patients and family members on proper healthcare management and disease prevention techniques
Collaborating with the patient's physician and other healthcare professionals to develop a care plan that meets the unique needs of each patient
Maintaining detailed and accurate patient records and documentation
Active LPN license in good standing
Minimum of 1-year experience in a clinical setting, preferably in home health care
Valid driver's license and reliable transportation
Ability to work independently and make sound decisions based on patient needs
Strong communication and interpersonal skills, with the ability to work with a diverse patient population
Knowledge of state and federal regulations governing home health care
CPR certification and basic life support training
