Sign-On Bonus 10K

• Agency: The Ohio State University Wexner Medical Center Home Care
• Schedule: Monday–Friday, 8:30 AM–5:00 PM
• Territory: Columbus – OSU/Columbus Area, Hilltop, Hillard, N. Univ district

Position Summary

Join a compassionate interdisciplinary home health team providing skilled physical therapy (PT) services to patients in their homes. You'll evaluate, develop, and implement individualized treatment plans to help patients restore function, improve mobility, and reduce hospital readmissions while supporting patients and families throughout their recovery journey.

Qualifications

• Current Physical Therapist (PT) license in Ohio
• Minimum 1 year of PT experience in an acute care setting
• Home health experience preferred
• Strong communication and clinical decision-making skills
• Valid driver's license and auto insurance

Key Responsibilities

• Complete evaluations, OASIS assessments, and individualized care plans
• Provide skilled PT services in patients' homes
• Educate patients and caregivers on treatment plans and equipment use
• Monitor progress and communicate changes with physicians and care team
• Supervise PTAs as appropriate
• Maintain timely, accurate documentation and regulatory compliance

Requirements

• Reliable transportation and ability to travel throughout assigned territory
• Ability to lift up to 50 lbs. with or without assistance
• Meet all health screening and employment requirements
