Patient Navigator (Internal)

The Patient navigator reports directly to the Medical Officer. The patient navigator is to understanding Cervical cancer program, will assist with linking customers to the clinical services, assist patients with scheduling appointments and follow ups, provide patients with information about their condition, offer emotional support to patients, etc. Specific duties include:-

Essential Duties

• Know and understands Cervical Cancer program
• Link clients to clinical services at the facility
• Ensure a safe, clean and organized conditions at all relevant service points
• Work with clients and actively link them between the facility and the community
• Assist patients with scheduling appointments, follow ups, tests and treatments, ensuring smooth transitions between different levels of care.
• Provide patients with information about their condition, treatment plans and available healthcare services.
• Educate patients on how to manage their health conditions effectively
• Offer emotional support to patients and their families by providing reassurance, guidance and assistance throughout their healthcare journey
• Maintain accurate and up to date records of patient interactions, care plans, and resource referrals
• Conduct outreach to educate communities about cervical cancer health care services and preventive care programs.
• Work closely with healthcare teams, including doctors, nurses, social workers, HSAs to coordinate comprehensive care for patients
• Prepare monthly work plans
• Providing support during clinics- counselling of clients, screening and other duties
• Submit daily, weekly, monthly and quarterly reports as needed

Requirements

Competencies and experience required

• Minimum of Malawi School Certificate of Education
• Community Development training or experience
• Strong interpersonal skills to engage staff and community members
• Fluent in written and oral Chichewa and English

Working condition and physical requirements

• The work will be in the field which may include early morning, evening and weekend assignments.
• The job is physically demanding.
