Care Coordinator

The Care Coordinator provides culturally grounded care coordination and case management for Relatives served through Kno'Qoti Native Wellness, Inc.'s (KNWI) Enhanced Care Management (ECM) and Community Supports (CS) programs in the River Valley Region, serving Glenn, Colusa, Butte, and Tehama Counties.

This position connects Relatives to healthcare, behavioral health, housing, cultural healing, and community resources while collaborating with tribal and community partners to advance KNWI's mission of healing, wellness, and connection.

This is currently a hybrid position, with work performed remotely and throughout the River Valley Region. The position will transition to KNWI's River Valley Regional Office in Willows, California, once the office opens.
