Job Description

• St. Gertrude’s is
• seeking a Minnesota licensed Social Worker to provide social service
• assessments, care planning, resident advocacy and relationship building with
• patients in our Transitional Care Unit. Candidates must demonstrate strong,
• positive, interpersonal skills, flexibility and the ability to work in an
• environment with rapid changes.
• This is a. 8 temporary position.

Qualifications

• Current social license
• 1 + years’ experience in Skilled Nursing

Facility

• Familiar with federal and state LTC regulations
• CPR certified
• Excellent verbal and written communication
• skills
• Able to read, write and communicate in English
