About The Role
Your Role as Case Manager – Youth & Health Education Service
In this position you will provide case management and facilitation services to program clients accessing our service. Community outreach across the Gold Coast is also a component of this role.
Key responsibilities of this position include
- Provide client-centred case management, advocacy and support to help young people achieve their goals and address their individual needs.
- Develop, implement and review personalised support plans in collaboration with clients.
- Deliver youth-friendly support, including emotional wellbeing guidance, information and referrals to appropriate services.
- Facilitate educational programs, workshops and health promotion activities that meet the needs of the target group.
- Build and maintain strong relationships with community services, stakeholders and partner organisations to support positive client outcomes.
- Coordinate access to support services and community resources, ensuring clients receive timely and appropriate assistance.
- Maintain accurate case notes, client records, reports and program statistics in line with organisational requirements.
- Respond effectively to crisis situations, conduct client assessments and work collaboratively within a multidisciplinary team to achieve program goals.
This is a part-time position consisting of 30 hours per week.

What We’re Looking For
- A completed degree in relevant social welfare matters and understanding of family relationship issues involving adolescents.
- Strong case management principals.
- Experience with intake, advice, advocacy and referrals.
- Commitment to social justice principals, confidentiality and privacy.
- Experience in establishing and maintaining agency network relationships within the human service environment to better the needs of young people.
Essential Requirements
- Proof of COVID-19 vaccination status (or as required by legislation and WMQ policy)
- Relevant probity checks required by legislation and WMQ Limited policy.
- Current Driver’s Licence and willingness to travel for work
