Role Overview

We’re hiring a compassionate Occupational Therapist to deliver high-quality, in-home care. You’ll help patients improve independence, function, and quality of life while collaborating with a supportive care team.

🔑 What You’ll Do

✔️ Assess patients and create individualized care plans

✔️ Provide therapy based on physician orders

✔️ Educate patients, families, and caregivers

✔️ Evaluate home safety and recommend equipment

✔️ Track progress and maintain clinical documentation

✔️ Collaborate with physicians and care teams

✔️ Train patients in daily living skills & adaptive equipment

✔️ Supervise COTAs and home health aides (as applicable)

✔️ Support discharge planning and continuity of care

✅ Qualifications

✔️ Licensed Occupational Therapist (state required)

✔️ Accredited OT degree

✔️ Home health experience preferred

✔️ Strong communication & independent work skills

✔️ Valid driver’s license & reliable transportation
