About The Role

This position reports to a Licensed Professional and Agency Director(s).

Education and Experience

Preventive professional" means, within the MHIDD/SAS system of care

  • An individual who holds a license, provisional license, certificate, registration or permit issued by the governing board regulating a human service profession, except a registered nurse who is licensed to practice in the State of South Carolina by the South Carolina Board of Nursing who also has four years of full-time accumulated experience in MHIDD/SA with the population served; or
  • A graduate of a college or university with a Master’s degree in a human service field and has one year of full-time, post-graduate degree accumulated MHIDD/SA experience with the population served, or a substance abuse professional who has one year of full-time, post­ graduate degree accumulated supervised experience in alcoholism and drug abuse counseling; or
  • A graduate of a college or university with a bachelor's degree in a human service field and has two years of full-time, post-bachelor's degree accumulated MHIDD/SA experience with the population served, or a substance abuse professional who has two years of full­ time, post-bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling; or
  • A graduate of a college or university with a bachelor's degree in a field other than human services and has four years of full-time, post-bachelor's degree accumulated MHIDD/SA experience with the population served, or a substance abuse professional who has four years of full-time, post-bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling.

Responsibilities

  • Development, monitoring and revision of person-centered plans and plans of care
  • Coordination of other services and supports related to maintaining an individual's health, safety, and well-being in the community
  • Facilitating person centered planning, team meetings,
  • Facilitating the service delivery process, including initial assessment, ongoing monitoring of care and services
  • Monitoring to assure quality care and continued appropriateness of services
  • Submitting person centered plans to Value Options or LME for authorization
  • Ensuring linkage to the most clinically appropriate and effective services
  • Supportive counseling to address the diagnostic and clinical needs of the recipient
  • Assistance with therapeutic interventions
  • Psychoeducation and training of family, unpaid caregivers,
  • Relapse prevention and disease management
  • Ongoing symptom monitoring and management
  • Service coordination activities within the established person-centered plan
  • Documenting service provision accurately and timely
  • Attending staff meetings
  • Quality improvement activities
  • Participation in continuing education
  • Perform NCI Training
  • Marketing
  • Assist the director
  • Other duties as assigned by supervisor or director
