About The Role

This opening is for a PRN RN position for both Home Care and the Heartford House locations.

Delivers quality hospice and palliative nursing care to patients in the inpatient hospice setting and/or the patient’s home. Acts as a patient/family advocate and an HPC representative, meeting the patient/family needs throughout the hospice experience.

The responsibilities of this position enumerated below, involve significant physical activities including standing, sitting, lifting (up to 50 pounds unassisted), bending, stooping, pushing, pulling, twisting, manual and finger dexterity and eye-hand coordination. May be exposed to infectious waste/diseases, to include but not limited to TB, AIDS, COVID, and Hepatitis B viruses. Must be willing to work beyond normal working hours, including weekends and holidays when necessary.

DUTIES

  • Assumes primary responsibility for a patient/family caseload that includes assessment, diagnosis, planning, implementation, teaching, and evaluation phases of the nursing process
  • Initiates communication with attending physicians, hospice staff members, and other agencies as needed to coordinate optimal care for the patient/family
  • Obtains data on physical, psychological, social, and spiritual factors that may influence patient/family health status and incorporate that data into the plan of care
  • Maintains up-to-date patient records
  • Other duties as needed

Must be a graduated of an accredited school of nursing, hold a current licensure in the state of Kentucky, and be CPR certified.
