About the Role

• We are seeking a Physician to provide on-site rounding consultation services in nursing homes/skilled nursing facilities, with a primary focus on treating
• a combination of acute and chronic pain syndromes.
• This role is ideal for Physicians who enjoy independent practice, value consistent schedules, and are comfortable managing complex, medically fragile populations.

What You’ll Do

• Conduct regular on-site rounds in assigned facilities
• Evaluate and manage acute and

chronic pain conditions

• Treatment via a multimodal approach as well as pharmacologic management, including controlled substances
• Coordinate care with facility staff (PT’s, RNs, DONs, administrators)
• Complete timely EMR documentation

Patient Population

• Acute rehabilitation and long-term care residents
• Geriatric patients with multiple comorbidities
• Chronic pain, mobility-related pain, and dementia-associated pain syndromes

Schedule & Work Environment

• Primarily weekday daytime hours
• No hospital shifts
• Travel between facilities is required in some cases
• Predictable patient census and rounding schedules

What We’re Looking For

• Clear and Active State Medical license
• DEA license (or ability to obtain)
• Experience or strong interest in:
• Geriatrics
• Long-term care
• Pain management
• Comfortable working independently and traveling locally
