About The Role

DescriptionPosition Summary

The Long Term Care LPN provides direct nursing care to residents under the supervision of a registered nurse, supporting individualized care plans that promote comfort, safety, dignity, and quality of life. This role contributes to ongoing assessment and monitoring, communicates changes in resident condition, and performs delegated clinical interventions in accordance with regulatory, safety, and facility standards. The Long Term Care LPN works collaboratively with CNAs, providers, and the interdisciplinary care team to support consistent, resident-centered care and continuity across shifts. Through professional clinical practice, accurate documentation, compassionate engagement, and strong teamwork, this position helps maintain a supportive, therapeutic environment within Trinity Homes.

Key Responsibilities

  • Assist the RN with assessments, data collection, and continuous monitoring of residents’ physical, emotional, and functional status.
  • Provide delegated nursing care, including medication administration, treatments, wound care, and chronic condition monitoring in alignment with resident care plans.
  • Document all assessments, interventions, and resident responses clearly and accurately in the electronic record to support regulatory compliance and continuity of care.
  • Communicate promptly with the RN, provider, and family regarding changes in resident condition and assist with updates to individualized care plans.
  • Supervise CNA staff during daily care activities, ensuring proper techniques, safety practices, and resident dignity are upheld.
  • Support a safe, clean, and homelike environment by following infection prevention, safety, and equipment handling procedures.

QualificationsLicenses and Certifications Required

  • Current or temporary LPN license in ND or compact LPN licensure.
  • BLS certification is required within the orientation period.

Educational Requirements

LPN program as evidenced by licensure.

Experience Requirements

Previous nursing home experience preferred, but not required.

Special Skills or Training Requirements

Good organizational and interpersonal skills.

Physical Requirements

The LPN frequently stands, walks, bends, and assists residents with transfers, repositioning, and activities of daily living. The role involves lifting or moving residents and equipment with appropriate assistance and may require pushing wheelchairs, carts, or beds. Adequate vision, hearing, fine motor skills, and clear communication are essential for providing safe and effective resident care.

Environmental Requirements

Work occurs within a long term care environment where residents may have chronic conditions, cognitive changes, or varying levels of dependency. The LPN may be exposed to blood and body fluids, cleaning agents, and frequent interaction with medical equipment. Adherence to infection control practices, PPE requirements, and safety protocols is essential. The role may involve responding to unexpected resident needs, behavioral changes, and periods of increased activity.
