About The Role

DescriptionPOSITION SUMMARY

The Hospice of Baton Rouge is seeking a dedicated and detail-oriented Full-Time The LPN Assistant, Quality Improvement, Regulatory Compliance & Education to help strengthen our culture of quality, compassion, and regulatory excellence. This position will partner with interdisciplinary teams to collect and analyze quality data, support regulatory readiness, coordinate educational programs, and promote continuous performance improvement to enhance patient care and ensure compliance with applicable federal, state, and accreditation standards.

JOB FUNCTIONS

Support the organization's Quality Assessment and Performance Improvement (QAPI) program through quality improvement, regulatory compliance, and performance improvement initiatives.

Conduct clinical documentation audits to ensure compliance with organizational policies, CMS, LDH, CHAP, and Hospice Quality Reporting Program requirements.

Collect, analyze, and report quality metrics and performance data, identifying trends and opportunities for continuous improvement.

Assist with investigations and reviews of quality indicators, adverse events, and other performance improvement activities.

Provide education and coaching to staff on regulatory requirements, documentation standards, and quality improvement best practices.

Collaborate with interdisciplinary teams to promote regulatory readiness, patient safety, and organizational excellence.

Provide patient care visits as needed under the supervision of a Registered Nurse and maintain accurate clinical documentation.

Support infection prevention, emergency preparedness, and other initiatives that promote a safe and compliant care environment.

QualificationsEducation

Diploma in Practical Nursing.

Licensure

Current Louisiana license from an accredited school of Practical Nursing.

Current CPR Certification

Experience

  • Minimum three years of recent varied work experience as a professional nurse, and preferably one of the three years in an acute, oncology, home health and/or hospice setting.
  • Knowledge of Medicare/Medicaid regulations and guidelines.

Skills

  • Ability to work independently as well as on an Interdisciplinary Team.
  • Demonstrated skill in nursing practice, ability to cope with emotional stress and tolerance of individual lifestyles
  • Must have good organizational skills and ability to prioritize effectively.
  • Knowledge in use of computers, copiers, and other office equipment
  • Good communication skills and techniques
  • Supportive and sensitive to needs of patients, families and physicians, as well as staff and volunteers

Technology

Proficiency in electronic health records (EHR) and other healthcare software.

Other Requirements

  • Must have own transportation and provide proof of liability insurance which meets state requirements.
  • Must have good communication skills and techniques with facilities, patients, families, office staff, and physicians.
