Job Description
- Full-time Transitional Care Navigator
- Schedule: Monday thru Friday, Full Time
- Excellent communication, negotiation and public relation skills.
- Ability to market effectively with customers, referral sources, and the community.
- Responding to customer and community needs regarding education as it relates to the hospice philosophy of care and delivery of services.
- Developing and increasing the access to Paradigm Hospice & Palliative Services within the service area.
- Assist, facilitate and coordinate between our referral sources, business development associates, our intake team and our patients and their families, the referral and admission process to promote a continuum of care model.
- Facilitate the consent and referral to admit process.
- Serve as a resource to review and provide available patient information related to a referral.
- Ensure coordination and communication with referral sources and hospice agency leadership once consents are completed.
- Serve as liaison between assigned hospital(s), skilled nursing facilities, community and hospice programs, with coordination of referrals/admission process and services.
- Promptly respond to customer, both internal and external, and referral source requests and concerns
- Compliance with accepted professional standards and practice.
- Self-directed with the ability to work with little supervision.
- Flexible and cooperative in fulfilling all obligations.
- Ability to be creative and "think outside the box" and have fun doing so.
- Great organization skills.
- Develop relationships in the community on behalf of the company.

Requirements
- One year Hospice experience preferred
- Current driver's license
- Must have reliable transportation and insurance
