Home Care Nurse with license

About The Role
We are seeking compassionate and dedicated Home Care Nurses with a valid DHA, DOH, or SHA License/Eligibility to provide high-quality nursing care to patients in their homes. The successful candidate will deliver patient-centered care, monitor health conditions, administer treatments and medications, educate patients and families, and ensure continuity of care while maintaining the highest standards of clinical practice.
Key Responsibilities
- Provide professional nursing care to patients in home care settings.
- Assess patients' physical, psychological, and social needs and develop individualized care plans.
- Administer medications, injections, intravenous therapy, and prescribed treatments as per physician orders.
- Monitor and record vital signs, patient progress, and response to treatment.
- Perform wound care, catheter care, tracheostomy care, PEG feeding, suctioning, and other nursing procedures as required.
- Assist patients with activities of daily living (ADLs), including personal hygiene, mobility, feeding, and rehabilitation support.
- Educate patients and family members on disease management, medication compliance, nutrition, and preventive healthcare.
- Maintain accurate nursing documentation and patient records.
- Recognize changes in patient conditions and promptly communicate with physicians and healthcare teams.
- Adhere to infection prevention and control protocols and patient safety standards.
- Ensure compliance with DHA, DOH, or SHA regulations and home healthcare policies.
- Maintain patient confidentiality and deliver compassionate, culturally sensitive care.
Requirements
Qualifications & Requirements
- Bachelor's Degree or Diploma in Nursing from a recognized institution.
- Valid Registered Nurse (RN) license in the home country.
- Valid DHA, DOH, or SHA License/Eligibility is mandatory.
- Minimum 2 years of clinical nursing experience , preferably in home healthcare, medical-surgical, ICU, emergency, or rehabilitation settings.
- Current BLS certification ; ACLS certification is an added advantage.
- Strong clinical assessment and patient care skills.
- Excellent communication, interpersonal, and documentation skills.
- Ability to work independently and travel to patients' residences.
