Home Infusion Pharmacist

This is an excellent opportunity for a pharmacist who enjoys clinical patient care, infusion therapy, and working closely with physicians, nurses, and healthcare teams in a collaborative environment.

Position Responsibilities

Review, verify, and dispense infusion medications accurately and efficiently
Collaborate with physicians, nurses, and clinical teams regarding infusion therapies and medication management
Monitor patient therapies and provide clinical recommendations when appropriate
Counsel patients and caregivers on medication administration and side effects
Participate in quality assurance, compliance, inventory management, and operational initiatives
Active Pharmacist license
Infusion, specialty pharmacy, or long-term care experience preferred
Strong clinical knowledge and attention to detail
Excellent communication and organizational skills
