Job Description

Schedule: Full Time Days

Hours: Monday -Friday 8:30am -5pm with occasional weekend coverage.

Location: Ormond Beach Office

Manages schedules for all patients and edits schedules for employees calling in sick, ensuring timely reassignment of patients. Completes requested schedules as tasks appear in workflow according to agency scheduling guidelines. Schedules discharge visits, OASIS collection, or recertification visits following case conferences when tasks appear on the action screen. Deletes remaining schedules after TIF OASIS collection. Reschedules declined or missed visits when appropriate. Processes reassigned and rescheduled visits. Ensures supervisory visits are scheduled according to agency protocol and regulatory mandates. Considers the entire episode of care and adjusts service codes as needed related to discharge visits and supervisory visits. Updates employee schedules daily, considering productivity and geographic issues, and discusses assignments with supervisors or managers for approval as directed. Assists with weekly coverage rotation to ensure scheduling functions are completed appropriately when staff are off. Facilitates communication between field staff, patients, and physicians to maintain proper care coordination and continuity of care. Maintains accurate records of all scheduled visits and communications. Performs other duties as assigned.

Field of Study

Work Experience

  • 5+ years of home health experience and a good understanding of staffing and scheduling requirements related to home health from a field position perspective. [Required]
  • Experience working in a Medicare certified homecare agency [Preferred]

Pay Range

$16.63 - $26.60
