Case Manager - Mental Health Program (Full-Time)

Position Summary

To provide case management services for a caseload of clients with severe mental illness who are homeless, at risk of homelessness or transitioning to independent living in the Mississauga/South Etobicoke area. Case Managers work in partnership with clients to develop the skills and supports they need to live successfully in the community. Clients are supported through visits in their own home, in the community or in the office.

The Case Manager works within a Trillium Health Partners community-based case management service for Mississauga residents, aged 18 and over, who have a history of serious and persistent mental illness. They work in partnership with clients to develop the skills and supports needed to live successfully in the community. The successful candidate will meet with clients either in their own home, in the community or in the office to provide recovery-based support.

Responsibilities

• Manage a full caseload, providing case management support to assigned individuals
• Complete assessment reports outlining individual psychosocial needs and determining eligibility for the program
• Engage individuals in the Recovery process through a goal oriented, skills-based approach
• Ongoing monitoring of individual’s wellness, assessment of risk and overall progress in one’s Recovery Process
• Liaise with existing treatment team including physicians
• Provide recovery-oriented case management and support to individuals experiencing first-episode psychosis or at risk of developing psychosis.
• Conduct comprehensive assessments and identify clients’ clinical, psychosocial, housing, financial, educational, vocational, and community support needs.
• Develop and regularly review individualized care and recovery plans in collaboration with clients, families, caregivers, and the interdisciplinary team.
• Build therapeutic relationships and support client engagement in treatment, recovery, and community-based services.
• Monitor clients’ mental health, functioning, treatment progress, and risk, and respond to changes in clinical status as appropriate.
• Provide psychoeducation to clients and families regarding psychosis, treatment, recovery, relapse prevention, and available resources.
• Coordinate care and facilitate communication among clients, families, healthcare providers, and community service agencies.
• Provide outreach and community-based support, including home and community visits, as required.
• Maintain accurate clinical documentation and complete required service activity, outcome, and program data in accordance with organizational standards.
• Participate in interdisciplinary meetings, case consultations, quality improvement initiatives, program development, and other activities that support the delivery of evidence-informed early psychosis intervention services.

Qualifications

• Bachelor’s degree or Community College diploma in a health-related discipline; relevant professional registration with a recognized regulatory college is preferred.
• Minimum of five (5) years of experience working with clients experiencing serious mental illness; experience with psychosis or early psychosis is preferred.
• Experience working with individuals experiencing first-episode psychosis, complex mental health needs, or significant psychosocial challenges is an asset.
• Experience working with individuals who are homeless, precariously housed, or accessing shelters is an asset.
• Experience supporting individuals with substance use concerns and/or concurrent mental health and substance use challenges is an asset.
• Strong knowledge of community resources and experience coordinating services and supports across health and social service systems.
• Knowledge and experience applying recovery-oriented, trauma-informed, and person-centred approaches to care.
• Demonstrated experience in assessment, treatment planning, client intervention, crisis intervention, and suicide risk assessment.
• Excellent communication, organization, presentation, documentation, and problem-solving skills, with demonstrated ability to work effectively both independently and as part of an interdisciplinary team.
• Proficient computer skills and ability to maintain accurate and timely clinical documentation.
• Valid driver’s licence, appropriate insurance coverage, and access to a reliable vehicle for community-based work.
• Demonstrated commitment to client-centred care, collaborative practice, and the principles of early intervention and recovery.
• Ability to speak a second language is considered a definite asset.
• Demonstrated positive attendance and reliability record.
