Community Health Worker (CBH BIGGS Alzheimer’s dept)

About The Role
We are looking for a CHW who is passionate about supporting families caring for individuals living with Alzheimer's disease and other dementias. The ideal candidate would have:
- Strong compassion and empathy
- Excellent communication and interpersonal skills
- Knowledge of community resources
- Familiarity with dementia care and caregiver support
Role Responsibilities
- Communicating with caregivers and families of individuals with dementia
- Providing resources, coaching, encouragement, and active listening
- Conducting home safety assessments
- Making outbound phone calls and electronic communications
- Completing occasional home visits
Education and Experience
- High School Diploma or GED is required.
- One (1) year related experience is required.
- Presents case studies from patient's perspective to primary care team in order to facilitate a patient-centered practice
- Performs outreach calls, engages in face-to-face encounters, performs home visits or other community-based visits when necessary
- Communicates relevant information to providers in a timely manner and documents appropriately in the electronic health record
- Participates in local community engagement activities with local agencies as appropriate
- Counsels patients with appropriate and relevant educational materials
- Identifies patterns of concern that surpass neighborhood capacity for health and engages diverse outside resources accordingly
- Enhances patient's individual and community capacity by growing health knowledge and self-sufficiency through a range of activities such as outreach, informal counseling, social support and advocacy
- Assists patients in connecting to appropriate health care information and services, addressing barriers to receipt of follow-up services as well as treatment
- Provides patient reminders to upcoming appointments and next steps of treatment
- Serves as liaison between patients, health/social services, and the community to enable access and improve the quality and competence of service delivery
- Performs all other duties as assigned
- Demonstrated knowledge of health and social services systems and familiarity with community resources and services.
- Ability to maintain full confidentiality of program participant information and data.
- Ability to shape others' understanding in ways that capture interest, informs, and encourages participation.
- Builds and maintains positive working relationships, with individuals and work groups through open communication and collaboration; works with others to accomplish goals and objectives.
- Proficiency in Microsoft Office Suite.
