Job Description

Full-time Hospice Admission RN

5 days per week
Territory: Counties surrounding Office Location
Report to Clinical Coordinator and/or Intake Nurse each shift to learn of assigned visits/tasks for that day. Common assignments may include:
Hospice evals or assessments
Urgent or change of condition visits
General visit coverage for a different RNCM when on PTO or when staffing demand is present
Tuck-in visits
GIP visits
Other misc. PRN visits.
Appropriate follow-up / coordination of care with the involved RNCM or other involved team members after completing a visit.

Requirements

• One year Hospice experience preferred
• 1 year of RN experience required

Licensed Registered Nurse in State of Indiana

• Current driver's license
• Must have reliable transportation and insurance
