About The Role

Liquid Personnel is recruiting Adult Social Worker for its client's Community Team at <Ealing.As> a Social Worker in the Adult Community Team, you will manage a varied caseload of adults with a range of social care needs. Working closely with individuals, families, health professionals, and community partners, you will promote independence, wellbeing, and informed choice while ensuring statutory responsibilities are <met.You> will play a key role in supporting vulnerable adults, undertaking assessments, coordinating care, and safeguarding those at risk.

What will your responsibilities be?

Manage a caseload of adults with varying levels of need, including complex cases depending on experience

Complete Care Act assessments, reviews, and reassessments

Develop and review person-centred support plans that promote independence and wellbeing

Undertake safeguarding enquiries and manage risk effectively

Complete Mental Capacity Assessments and apply the Mental Capacity Act where appropriate

Support hospital discharges and work proactively to prevent unnecessary admissions

Work collaboratively with health services, housing providers, voluntary organisations, and other partner agencies

Provide information, advice, and guidance to adults and their carers

Maintain accurate, timely, and high-quality case records

Contribute to service development and continuous improvement initiatives

Support and supervise less experienced colleagues where appropriate

Qualifications & Experience

To be successful in this role you must have,

A recognised Social Work qualification

Current Social Work England (SWE) registration

Experience undertaking assessments, reviews, and safeguarding interventions within adult social care

Good knowledge of the Mental Capacity Act and relevant legislation

Strong multi-agency partnership working skills

Excellent communication, organisational, and problem-solving abilities

A compassionate, proactive, and person-centred approach to practice
