About The Role
**Please note, as part of our hiring process, you must attend a 2-week paid orientation. This will be Monday-Friday, 8am-4pm, regardless of your future shift availability**
Summary of Position
Community Resource Professional assists individuals with optimizing their personal, social, and vocational opportunities within the community. Examples of this includes but are not limited to: leisure and recreational activities, volunteerism, self-advocacy, cultural and spiritual activities, and use of public transportation.
Leadership Responsibilities
- Advance Tangram’s mission and values.
- Builds positive relationships with clients, teams, professionals, and members of the community.
- Demonstrates the ability to work with autonomy and manage work hours effectively.
Duties/Responsibilities
- Collaborate with persons served, supervisors, and other team members to identify community interests and goals.
- Work with the client’s team to update the community goals as needed.
- Help individuals maximize community involvement in areas of recreation, socialization, self-advocacy, and other life skills.
- Assist individuals in the development of meaningful connections by acting as a “bridge builder” to establish greater ties to the community and to support the development of friendships.
- Identify community resources that could enhance the individual’s quality of life and meet client goals.
- Can work flexible hours in various community settings.
- Provides transportation for clients in a personal vehicle.
- Participates in client meetings either directly or by report, to provide updates on community activities.
- Responsible for understanding and implementing Behavior Support Plans (BSP) if applicable.
- Responsible for understanding and implementing all medical High Risk Plans (HRP) if applicable.
- Documents work thoroughly and timely.
Requirements
Education & Experience
- High school diploma or GED equivalent
- Experience in a human service field and/or experience with individuals with developmental or other disabilities preferred
- Must be 18 years of age or older
- Must have a valid driver’s license, acceptable driving record, meet agency’s driver insurability requirements, reliable vehicle, and up to date automobile insurance. If position involves driving agency vehicles, must be 21 years of age.
- Must have basic computer skills
- Must own a smart phone for purposes of required documentation. Access to a desktop, laptop, or tablet is optional
- Must maintain CPR and first aid certification
- Must pass Medication Administration and stay current on all other training as defined by state, accreditation body or agency guidelines
Physical Requirements
- This position may requires significant bending, lifting, and stooping
- Alternating periods of sitting or standing as dictated by client needs
- Must be able to lift/transfer individuals and physically assist clients with movement and care
- Ability to lift up a minimum of 35 pounds
- Repetitive physical activity
