Position Overview
The Neurosciences Department is seeking a Registered Nurse Navigator to support our growing Traumatic Brain Injury (TBI) Program. Working closely with a TBI Physiatrist and multidisciplinary care team, this role serves as a key resource for patients and families, ensuring seamless care coordination throughout the recovery journey.
The Nurse Navigator acts as a patient advocate, helping guide patients through complex treatment plans while supporting exceptional clinical outcomes and patient experiences.
Key Responsibilities
- Coordinate patient care from referral through treatment, follow-up, and transition of care.
- Serve as a primary point of contact for patients, families, and caregivers.
- Educate patients and families regarding diagnoses, treatment plans, and available resources.
- Collaborate with physicians, nurses, therapists, case managers, and other team members to ensure timely, coordinated care.
- Identify barriers to care and connect patients with appropriate support services and community resources.
- Track and monitor program metrics, patient outcomes, and satisfaction measures.
- Assist with quality improvement initiatives and program development efforts.
- Support communication with referring providers and community partners.
- Perform other duties that contribute to the growth and success of the TBI Program.
Qualifications
- Requires an Associates of Nursing (ASN).
- Bachelors of Nursing (BSN) preferred.
- Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license.
- Requires that the RN has graduated from a nationally accredited nursing program. 3-5 years of experience preferred.
- Requires Basic Life Support certification through the AHA.
- Other advanced life support certifications may be required per unit/department specialty according to patient care policies.
- Specialty certifications may be required per unit/department specialty in accordance with the department provision of care.
- Requires knowledge of personal computers using various types of word processing and database software.
- Requires clinical work experience, medical terminology.
- Requires understanding of medical record requirements, regulations and policies.
