About The Role

The Care Coordination Service is seeking a Nurse Practitioner to function as a Clinical Coordinator in the Post-Discharge Follow Up service, functioning under the 1115 Medicaid Waiver. Responsibilities include:

Assisting with facilitating bedside touchpoints for patients slated for discharge

Directing the Post-Discharge Coordinator and Scheduling Coordinator to efficiently maintain workflow and contact with patients prior, during, and after clinic visit (if required)

Conducting a mixture of virtual and in-person visits to capture patients that are post-discharge from WHMC—both inpatient and from the Emergency Room

Performing relevant physical exams and evaluating patients, documenting pertinent positive and

negative findings.

Diagnosing and treating disease and providing education regarding discharge instructions, medical management, and follow-up appointments

Evaluating patients to determine the necessity of sooner follow-up, if needed

Prescribing medication in accordance with New York State and federal law.

Ordering appropriate laboratory, radiological and special examinations or tests; interpreting results.

Providing preventative health services, including health education and counseling with the goal of preventing 30-day readmission

Collaborating with other clinicians and staff members to ensure continuity of care of patients; developing metrics and tracking outcomes pertinent to patient experience

Assisting with facilitating the Primary Care Nurse Practitioner Fellowship Program (teaching, curriculum development, and accreditation).

Master of Science degree in Nursing—Adult-Gerontology Primary Care Nurse Practitioner (preferred) or Family Nurse Practitioner, Doctor of Nursing Practice preferred

Completion of Primary Care Nurse Practitioner Fellowship Program

Graduate of a Nurse Practitioner education program registered by the New York State Education Department

Certification as an Adult-Gerontology Primary Nurse Practitioner or Family Nurse Practitioner from a New York State approved accrediting body – American Nurses Credentialing Center (ANCC), American Association of Nurse Practitioners (AANP).

Must possess current, unrestricted New York State Registered Nurse license and Nurse Practitioner license

Must possess active Drug Enforcement Agency (DEA) registration or be eligible for registration

Must possess active Medicare/Medicaid Enrollment or be eligible to enroll

BLS or ACLS certification required

Bilingual Spanish (preferred)
