Senior Patient Care Coordinator

Role Overview:We are seeking a dedicated Senior Patient Care Coordinator to join our reputable healthcare team. This position plays a crucial role in the coordination and delivery of patient care, ensuring that our patients receive the highest quality of services.Key Responsibilities:Coordinate with healthcare professionals to assess, plan, and implement patient care plans.Monitor and evaluate patient progress, adjusting plans as necessary.Manage patient records and ensure compliance with healthcare regulations.Educate patients and their families on treatment plans and care procedures.Required Skills and Experience:Must have a current and valid Registered Nurse (RN) license. A minimum of 5 years of nursing experience, preferably in a supervisory or coordinative role. Experience with electronic medical records.Preferred / Nice-to-Have Skills:Palliative care experience and a background in patient advocacy are <advantageous.Education> and Certifications:Bachelor's degree in Nursing (BSN) is required; a Master’s degree in Nursing or Healthcare Administration is preferred.Soft Skills and Behavioral Expectations:Strong communication and interpersonal skills.
