Key Highlights - Weekly pay: $2,027–$2,161 per week - Guaranteed hours: 40 per week - Contract duration: 13 weeks - Shift: 3x12-hour days - Start date: 08/31/2026 - Setting: Critical Access Hospital Responsibilities - Plan and conduct comprehensive patient evaluations and develop individualized treatment plans to restore and improve physical function. - Administer therapy interventions using established protocols and techniques, including appropriate modalities and therapeutic exercises. - Co-lead interdisciplinary patient groups, collaborating with other clinicians to achieve care goals. - Document evaluations, progress notes, and discharge plans accurately and within established deadlines. - Supervise assigned therapy assistants and coordinate care with the broader care team. - Contribute to quality and safety initiatives and ensure compliance with applicable policies and regulations. Requirements - Doctor of Physical Therapy (DPT) degree; required if licensure was obtained after January 2016, or foreign equivalent. - Physical Therapy licensure in the state of practice (California) or licensure in the state of practice as applicable. - CPR/BLS certification from an approved vendor (AHA-BLS or HS-BLS or RQIBLS) as required by policy. - Valid Driver’s License and age 21 or older. - At least two years of recent experience in the field; plus one year of home health experience. - Must know HCHB and OASIS as applicable to the assignment. - Education and licensing requirements as specified above.
