Community Support Specialist - Adapt

In this role, you’ll provide personalized, community-based support — meeting clients where they are: in their homes, in the office, and out in the <community.You>'ll help individuals set and achieve recovery goals, develop daily living skills, access vital services, and build supportive relationships — all while working as part of a collaborative, integrated care team.
