About The Role

We are currently hiring a Bilingual RN Care Manager to support members with chronic and complex health conditions through telephonic care management. You will assess and evaluate patient needs, develop care plans, and collaborate with interdisciplinary teams. Ideal candidates are bilingual RNs with strong clinical knowledge in chronic condition management.
