Job Description

• Lafayette, IN | Flexible Options
• As a Home Health RN, you’ll provide one-on-one care in the patient’s home. No call lights. No multiple patient assignments. Just meaningful care where it matters most.

• ✔ Friday – Sunday
• ✔ Saturday – Sunday
• No home health experience? We will train you.

What You’ll Do

• ✔ Complete Start of Care and follow-up visits
• ✔ Assess patients and update care plans
• ✔ Educate patients and families
• ✔ Coordinate with physicians and care team
• ✔ Document visits accurately

What You Need

✔ Active RN license

✔ 1+ year of nursing experience

✔ Reliable transportation and valid driver’s license

✔ Willingness to travel within Lafayette area

✔ Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
