Reports to: Manager
- Wage Range: $45.85 - $57.31 per hour.
- Hours: 37.5 Hours per Week
- Shifts:  Monday to Friday
- Status: Temporary Part-Time (End Date ~December, 2027)

As an integral member of the clinical team, the Patient Resource Coordinator , plays a vital role in ensuring the smooth and efficient daily operations of the Outpatient Transplant clinic.  The principal responsibilities include:
- Rooming patients in examination rooms
- Measure and record height, weight and vital signs
- Conduct basic nursing assessments; collect lab specimens and swabs
- Perform wound care as needed
- Educate and instruct patients and family members on relevant health topics
- Review and document patient medications (medication reconciliation)
- Perform cross-functional and/or other duties consistent with the job classification, as assigned or requested.
- Ensures the day-to-day transplant clinics are running efficiently while providing quality patient-centered care across various clinics
- Triages patients in clinic, prioritizing clinic needs in collaboration with medical staff and        operations manager/PCC
- Applies knowledge of nursing process, as it relates to scope of practice, to achieve identified patient goals
- Identifies and recognizes abnormal or unusual patient responses and consults with the nurse coordinator or MRP in clinic
- Ensures practice is consistent with professional standards and the ethical framework as outlined by the College of Nurses of Ontario and the UHN policy and procedure
- Demonstrates commitment to professional life long learning by participating in continuing education activities to meet the needs of the patient population served and the standards of the profession Performing reflective practice, analyzing and evaluating nursing knowledge and modifying practice accordingly to improve the quality of nursing and patient care
- Performs cross-functional and other responsibilities, as assigned and/or requested.
- Liaises with relevant parties, i.e., Physician, nurse coordinators, other allied staff as needed
- Communicates/coordinates/collaborates with Nurse Managers on opportunities or issues in the clinic setting
- Completion of college, or other approved Registered Practical Nursing program including practicum experience.
- Current registration with the College of Nurses of Ontario as a Registered Practical Nurse.
- Current Basic Cardiac Life Support (B.C.L.S.) certification.
- Current medication certificate which meets the standards of practice established by the College of Nurses of Ontario.
- Demonstrates competencies as per unit/program competency-based checklist.
- Recent experience working on a Transplant unit preferred.
- Ability to perform duties in a professional and courteous manner and produce high quality work while meeting deadlines in accordance to UHN standards.
- Exercise initiative and good judgment with ability to multi-task.
- Compliance with confidentiality requirements.
- Effective organizational, interpersonal and communication and time management skills.
- Knowledge of applicable legislative, UHN and/or departmental policies.
- Client service oriented, with the ability to effectively work with diversity and appreciate that people with different opinions, backgrounds and characteristics bring richness to the challenge or situation at hand.
- Good computer skills.
