Job Description
- Full-time Hospice LPN position
- Schedule: Monday thru Friday, 8a-5p
- Available territory openings: LPN to cover patients in 1-2 counties surrounding office
- Provides skilled and palliative nursing in the home as ordered on the plan of treatment and nursing care plan.
- Evaluates the significance of clinical data findings and communicates any pertinent information about the patient’s clinical status and ongoing needs to others in a timely fashion.
- Carries out the pain management and symptom management plan to keep the patient as comfortable as possible. Ability to adequately evaluate and re-evaluate pain.
- Utilizes appropriate pain management techniques.
- Educates the patient and family regarding pain management. Notifies the RNCM when pain is not manageable on the current medications.
- Demonstrates knowledge of medications and their correct administration based on age of the patient and patient’s clinical position.

Requirements
- Licensed Practical Nurse in State of Indiana
- Proof of current classroom CPR certification
- Hepatitis profile and TB vaccination record
- Current driver's license
- Must have reliable transportation and insurance
