The Justice Program Coordinator will drive the initial stages of care, focusing heavily on processing incoming referrals in a timely, customer-service oriented manner, participating in warm hand-off meetings to align with multidisciplinary care teams, and seamlessly scheduling intake and follow-up appointments.

Essential Duties and Responsibilities

Referral Management & Processing

Efficiently receive, review, and process incoming referrals across all 37 WA Justice Contract collaborations, verifying eligibility and ensuring swift entry into the program. Thoroughly documenting all collaboration with the health plan into mMR to display compliance with Justice intake requirements.

Warm Hand-off Coordination

Attend and actively participate in warm hand-off case management meetings with justice partners, care teams, and community providers to ensure smooth care transitions for members entering behavioral health services. Entering compliant and thorough documentation of members discussed into mMR.

Intake & Follow-up Scheduling

Serve as the primary scheduler for member intake assessments and ongoing follow-up appointments, managing complex calendars and reducing no-show rates through direct engagement.

Multidisciplinary Care Alignment

Collaborate directly with Program Directors, Care Extenders, internal Population Health Coordinators, and external health plan/justice partners to streamline workflows and maintain clear lines of communication.

Inbound & Outbound Communication

Handle high-volume communications via phone and email, serving as a primary liaison between justice-involved members, care teams, and external stakeholders with high empathy and professionalism.

Data Management & Tracking

Maintain accurate referral logs, tracking enrollment metrics, appointment completions, and care coordination efforts within Electronic Health Record (EHR) systems and tracking sheets.

Resource Curation

Maintain and update a centralized database of community resources specifically tailored to justice-impacted populations (e.g., re-entry services, behavioral health, housing support).

Cross-Program Coverage

Provide cross-coverage for other SPHM Program Coordinators during peak volumes or absences as needed.

Education & Experience Requirements

Education

Bachelor’s Degree required (Degree in Human Services, Social Work, Criminal Justice, Healthcare Administration, or related field preferred).

Technical Skills

Proficiency in Google Workspace, Microsoft Excel/Sheets, and data analytics or referral tracking tools.

EHR & HIPAA

Prior experience navigating Electronic Health Record (EHR) systems within a strict, HIPAA-compliant healthcare environment.

Justice System / Behavioral Health Interest

Strong background, knowledge, or deep interest in justice-involved populations, re-entry initiatives, and behavioral health/substance use support.

Organization & Scheduling

Superior organizational and multi-tasking skills, with proven experience managing high-volume scheduling across complex, multi-site programs.

Communication

Exceptional verbal and written communication skills with a strong foundation in empathetic, trauma-informed customer service.

Adaptability

Ability to thrive in a fast-paced, high-urgency remote environment while maintaining accuracy and accountability.

This is a 100% remote role that must be located in the United States.
