About The Role
Founded as Boulder Hospice in 1976, TRU Community Care is a nonprofit healthcare organization serving Boulder, Denver, and surrounding counties. With a mission to affirm life at every step of the journey with illness and loss, TRU provides a continuum of care through TRU Hospice, TRU PACE (Program of All-Inclusive Care for the Elderly), TRU Palliative Care, TRU Grief Services, and Landmark Memory Care. Title: Admissions Intake Coordinator Pay: $20.33 to $25.41 per hour Schedule: Monday-Friday, 8am to 5pm JOB SUMMARY The Intake Coordinator is responsible for assuring a prompt, courteous response to patient/family and referral sources’ requests for services. The Intake Coordinator facilitates the intake and admission process in a manner that promotes a team approach to care, respects patients/families, meets Medicare/Medicaid conditions of participation and accreditation requirements and reflects the mission and philosophy of TRU Community Care. Supports and assists admissions RNs with non-clinical administrative tasks to complete an admission. EDUCATION AND QUALIFICATIONS • Previous experience in a medical administrative role • 3+ years of hospice intake experience, highly preferred • Previous experience supporting patients/families in a health care setting • Degree in health services preferred • Excellent interpersonal and verbal/written communication skills required to effectively respond to all callers ESSENTIAL DUTIES • The first point of contact for external clients and referral sources seeking information and/or to schedule admission conversations; interfaces with both internal and external customers including physicians, discharge planners, long term care facilities as well as patients and family members. • Receives all incoming referrals and requests supportive documentation to facilitate the admission. • Prioritizes and triages calls, schedules referral and admission visits for Admissions Department staff. • Responsible for initiating and updating patient referrals by documenting patient information in EMR and any other required database. • Accurately inputs required demographic /clinical/financial information and documentation into electronic medical record. • Obtains pertinent medical records to facilitate timely admission. • Coordinates and collaborates with internal departments to assure timely admissions and visits including required physician services, care center availability, and team assignments including both palliative care and hospice.
