Director (RN) Case Management

We are seeking a candidate with 2–5 years of Case Management leadership experience (preferred) and a bachelor’s degree (required). The ideal candidate will be goal-oriented, self-driven, and demonstrate strong verbal and written communication skills. Proficiency with computers and Microsoft Office applications is also expected. This role will oversee both the Case Management and Spiritual Care departments. Key responsibilities include ensuring safe and effective discharge planning for all patients, maintaining Case Management policies and procedures, and providing strong leadership to support length-of-stay (LOS) goals and other organizational initiatives.
