About The Role

The physical therapist works under the supervision of leadership. Evaluates, plans, and implements treatment programs for the rehabilitation of patient, as directed by a physician, using approved treatment procedures. Provides patient/family education. Participates in family conferences. Responsible for all necessary documentation.

Provides physical therapy services to patients according to a written physician’s plan of care. Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences. May train patient in the use of prosthetic device. Identifies patient and family/caregiver needs for other home health services and refers as necessary. Prepares and submits clinical and progress summaries based on attainment of goals. Participates in discharge planning Provides a physical therapy consultation to home families/caregivers when indicated. Participates in Interdisciplinary Group to define and update the comprehensive plan of care. Supervises Physical Therapy Assistants according to organization policy and state regulations When therapy is the only skilled service, instructs, supervises and evaluates home health aide care every two weeks. Other duties as assigned Education Graduate of accredited physical therapy program - Required Work Experience 1 Year - Appropriate experience as a physical therapist - Required 1 Year - Community/home health experience - Preferred Licenses and Certifications Licensed Physical Therapist in State of Missouri - Required within 60 Days Basic Life Support (BLS) - Required within 90 Days Driver's License - Valid In State - Required Upon Hire Travel Requirements Must be able to travel between various system facilities and off-site locations as needed. - Required

Skills and Abilities Essential Technical/Motor Skills Ability to communicate clearly to patients, fine motor skills required. Interpersonal Skills Displays a friendly, positive attitude and communicates with patients, families, physicians and co-workers in a professional manner. Ability to communicate both verbally and written. Essential Physical Requirements Must move, kneel, crouch, stoop, reach, stand, push, pull, and lift. Must follow good body mechanics. Essential Mental Abilities Assess, evaluate, treat, and educate patients. Must be adaptable to the many challenges of differing home environments. Also includes explaining, forecasting, organizational skills and time management. Essential Sensory Requirements Visual, hearing and feeling skills. Exposure to Hazards Frequent exposure to communicable diseases, toxic substances, body fluids, road and weather hazards. Other Skills and Abilities Must be registered and successfully pass background check with the Family Care Safety Registry and other applicable state registries. Must be a licensed driver with an automobile that is insured in accordance with the state and/or organization requirements and is in good working order.
