You will collaborate with the Nurse Manager, Director, Physicians, and staff to ensure high-quality, cost-effective patient care with strong safety and satisfaction outcomes. The position emphasizes leadership, staff supervision, and coordinated care planning for patients and families. Key Highlights - Weekly pay: $1,807–$1,828 per week - Guaranteed hours: 36 hours per week - Contract duration: 13 weeks - Shift: 3x12 Nights - Start date: 08/03/2026 - Setting: Home Healthcare - Hospice Responsibilities - Provide unit-based leadership and coordinate care with the Nurse Manager, Director, physicians, and staff. - Supervise and direct nursing and ancillary personnel; assign tasks and monitor performance. - Initiate and coordinate conferences with health team members, patients, and families to develop or revise individualized care plans. - Direct and monitor patient care activities to ensure safety, quality, and timely interventions; participate in patient care as needed. - Foster patient- and family-centered care with attention to cultural diversity and language needs. - Conduct rounds to assess patient needs, gather feedback on nursing services, and drive continuous quality improvement.
