Key Highlights - Weekly pay: $1,950–$2,093 per week - Guaranteed hours: 36 hours per week - Contract duration: 13 weeks - Shift: Evenings (3:00 PM–11:00 PM) - Start date: 08/24/2026 - Setting: Home healthcare – hospice - EPIC experience required Responsibilities - Deliver coordinated nursing care for an individual patient or assigned group of patients using established standards, the nursing process, and evidence-based practice. - Collect relevant health data, analyze assessments, diagnose care issues, and develop a care plan with prescribed interventions. - Implement the plan, coordinate care delivery, and promote a safe, healing environment; monitor patient progress and adjust plans as needed. - Supervise and direct activities of various levels of nursing staff; collaborate with interdisciplinary team members to optimize patient outcomes. - Participate in discharge planning and ensure continuity of care; provide education to patients and families regarding conditions, procedures, and home care plans. - Maintain accurate documentation in the electronic health record (EPIC); communicate changes in patient status to the treatment team.
