Program Manager Advance Illness - Hospice of Cincinnati

The Program Manager of the Advanced Illness programs is responsible for the day-to-day operations, performance, and growth of HOC Navigators’ Palliative Care programs, including hospital inpatient, ambulatory, home-based, assisted living/long-term care programs, and the Advance Care Planning program. Seeking skilled leader to oversee key care delivery processes, including referral, admission, care coordination, and patient transitions, while ensuring collaboration across internal teams and external partners. Experience in palliative care or hospice preferred.
