Signature Albany Home Health is seeking a compassionate and dedicated

Licensed Practical Nurse (LPN) to provide quality care to patients in the comfort of their homes. If you're looking for meaningful one-on-one patient care, flexible scheduling, and a supportive team environment, we'd love to hear from you.

Position Summary

Under the supervision of a Registered Nurse, the LPN provides direct patient care, implements individualized care plans, administers treatments, educates patients and families, and collaborates with the care team to support positive patient outcomes.

Essential Responsibilities

• Provide skilled nursing care in accordance with physician orders and plan of care
• Administer treatments and monitor patient responses
• Document patient care accurately and timely
• Educate patients and caregivers on care plans and disease management
• Communicate changes in patient condition to the RN Case Manager
• Collaborate with the interdisciplinary team to ensure quality patient care

Qualifications

• Graduate of an accredited Practical Nursing program
• Current Oregon LPN license
• Home health, community health, or medical/surgical experience preferred
• Current CPR certification
• Strong communication and organizational skills
• Valid driver's license, reliable transportation, and current auto insurance
