Social Service Assistant - Case Management

The Social Services Designee is responsible for assisting residents and their families with social, emotional, psychosocial, financial, and discharge planning needs in accordance with federal regulations, California state requirements, and facility policies. The SSD serves as an advocate for residents' rights and quality of life while promoting resident-centered care throughout the facility.

The Social Services Designee works collaboratively with the Director of Social Services to assess resident needs develop individualized care plans, facilitate safe discharges, and support residents in achieving their highest practicable physical, mental, and psychosocial well-being.
