Hospice Referral Specialist – RN (Care Access Center)- Full Time, Days, 4 shifts per week, Atlantic Health Visiting Nurse Home Care Overlook/Morristown/Chilton Medical Center

This is an outstanding opportunity for an experienced Home Care or Hospice RN who enjoys both the clinical and relationship-building aspects of nursing. You'll be the clinical expert helping determine the most appropriate level of care for patients transitioning from the hospital while working closely with physicians, case managers, and families. It's a Monday through Friday daytime position where you'll truly influence patient outcomes before care even begins.
