HSCSN Caregiver Navigator

About the Role: The HSCSN Caregiver Navigator is a specialized support professional who acts as a guide, advocate, and peer mentor for families for children with special healthcare needs including behavioral challenges, developmental disabilities, and/or significant emotional and social challenges. They work to reduce family and caregiver stress by providing information, education, and direct linkage to community resources. These navigators have lived experience as caregivers, allowing them to provide compassionate support and empower families to navigate complex medical, educational, and social service systems. The navigator will work in conjunction with the care management team under the supervision of the care management leader. They will receive assigned cases based on the needs of the enrollee and work directly with the enrollee and families. They will interact with providers, social agencies, schools, community supports, and all of the HSCSN team members. They will use the EHR to document all interactions with the enrollee.

What you'll bring

Minimum Education

• High School Diploma or GED (required)
• Associate's degree preferred
• Bachelor's Degree preferred

Minimum Experience Required

2 years' Experience working as a caregiver for a special needs person/child in the community or at home (Required)

4 years' Experience working as a caregiver of special needs child in the community or at home and working directly with community agencies/social supports including the Foster Care System, Early Intervention Services, Behavioral Health Services, Clinical support services (Preferred)

Required Skills/Knowledge

• Broad clinical knowledge, customer service skills, experience in health care setting preferred.
• Experience in community-based health care programs
• Supports enrollees through advocacy and prevents escalation and to promote greater communication between families and care teams.
• Must be enrollee- and family-centered, someone that cares about people and wants to make a difference.
• Must have an understand of health care systems including the continuum of services that are offered for children with special needs
• Ability to learn and become expert in all community-based resources that support physical, behavioral, and social needs of children and families
• Ability to work both independently and within a team to drive the best clinical and social results for enrollees and their families
• Supports the mission and vision of the organization to serve the enrollees by providing the best care, at the right time in the right place at the right time
• Understands the basics of how the financial system balance the care and the costs to serve the enroll

The disclosed salary range includes the minimum and maximum rates within which Children’s National believes an individual’s base pay rate will fall for this position. It is not typical for an individual to be hired at or near the maximum of the pay range. The exact pay rate for this position will be based on a variety of factors in alignment with the Children’s National compensation philosophy. These factors are legitimate and non-discriminatory including, but not limited to, the current market conditions; organizational needs; the individual’s combination of prior work experience, level of education, knowledge, skills, and other qualifications.
