- Our centralized team of Clinical Quality Managers support many HER back-office clinical
- functions. This role is designed to help ensure accurate documentation, timely workflow
- completion, regulatory compliance, and high-quality patient care. By providing centralized
- clinical support, Clinical Quality Managers help reduce administrative burden on branch
- teams so local clinical leaders can focus on patient care and clinician development.
- Position Type: Remote, centralized position supporting multiple branch clinical field
- teams.
Position Overview
- The Clinical Quality Manager is a centralized, remote RN role that provides clinical and
- administrative support to field clinical teams. This is not a field position—the Clinical
- Quality Manager works remotely to help manage EMR workflows, order processing, and
- compliance-related activities.
- This role serves as an important partner to branch clinical leadership by helping ensure
- clinical documentation, order processing and workflows are completed accurately and on
- time. Success in this role requires strong attention to detail, sound clinical judgment, and
- comfort working in a fast-paced, technology-driven environment.
What You'll Do
Support Quality, Safety, and Compliance
- Help identify recurring documentation and workflow opportunities for improvement
- Support quality assurance activities by ensuring clinical work meets organizational
standards
Order Management & EMR Workflow Support
- Review and process physician orders within the EMR
- Ensure verbal orders meet documentation requirements
- Confirm orders are complete and aligned with patient care needs
- Support intake and referral processing through clinical review as needed
End of Episode Management
- Review discharge reports to ensure required documentation is complete
- Process and lock transfer to inpatient facility and death at home OASIS visits
- Verify discharge summaries and signed orders are present
- Confirm NOMNC documentation is completed when applicable
- Process discharge-related orders as needed
Hospital Hold Process Support
- Manage hospital hold documentation and inpatient transfer workflows
- Assist with plan of care updates for hospitalized patients
- Support resumption of care workflows and monitor pending approvals
- Complete discharge workflows for patients who remain in facilities at the end of an
episode when appropriate
Quality & Compliance Monitoring
- Review infection control reports and escalate concerns as needed
- Monitor wound score documentation and identify variances
- Review quality improvement events and communicate trends to leadership
- Process clinical alerts and potentially avoidable event notifications
- Audit missed visits and related documentation to support quality care delivery and
reimbursement compliance
- Monitor for timely initiation of care concerns
Patient Status Monitoring
- Review vital sign alerts and ensure appropriate follow-up documentation is
completed
- Monitor coordination notes for physician updates and clinically significant
medication-related concerns
- Review EMR dashboards for changes in patient condition and escalate issues
appropriately
Who You Are
- A detail-oriented RN who enjoys working within systems and processes to support high-quality patient care. You are organized, dependable, and comfortable managing multiple
- workflows while maintaining accuracy. You work well independently and communicate
- effectively with clinical teams when support or escalation is needed.
Requirements
Required
- Active RN license
- Prior home health field experience
- Minimum of 1 year of experience working in Homecare Homebase (HCHB) back office workflows
- Strong understanding of home health documentation and regulatory requirements
- Strong organizational skills and attention to detail
- Ability to manage multiple priorities in a remote work environment
- Effective written and verbal communication skills
Preferred
- Multistate/Compact RN License
- Willingness to seek additional state licensure
- Previous home health management experience
- Experience supporting multiple locations or clinical teams remotely
Compensation
Salary Range: $80,000 - $85,000 annually, dependent on experience (D.O.E.)
