About The Role

A minimum of level 5 Qualification in Healthcare, Childcare or any relevant course. 39hours/week
Provide support to service user in a placement/residential setting.
Liaise with other multidisciplinary team.
Support Service user with meaningful and relevant daily activities.
Planning, implementation and evaluation of service user care plan.
Required to send daily report to the team leader.
