James River Home Health is actively seeking a compassionate and highly committed Full-Time Occupational Therapist to join our growing team in Lynchburg, VA ! You will be responsible for providing skilled Occupational Therapy to clients in their homes on an intermittent basis. You will also coordinate with the interdisciplinary team to ensure an optimal care experience for the patient!

Salary: $83,700 - $117,000

A brief overview of Responsibilities

  • Assist the physician in evaluating the client’s level of function by applying diagnostic and prognostic procedures
  • Conduct ongoing assessments of the client’s needs, including outcome and assessment information set (OASIS) assessments
  • Evaluate the client’s significant other(s), and the home situation to determine what instruction will be required and what family assistance will be available to care for the client, as well as what other agency and community services will be required
  • Assist in the development and implementation of the interdisciplinary client care plan to include Occupational Therapy in consultation with the physician
  • Teach, supervise, and counsel the family and client in the total Occupational Therapy program and other related problems of the client at home.
  • Confirm, every week, the scheduling of visits with the Director of Therapy to coordinate agency visits
  • Prepare clinical/progress notes on the day of the visit and incorporate them into the chart within 24 hours.
  • Other duties as assigned

Qualifications

  • Must be a graduate of an approved Occupational Therapy school
  • Must be licensed in Virginia
  • Must have at least one year of institutional experience
