Care Manager RN Per Diem Day

Care Manager RN at Providence Redwood Memorial Hospital. This position is Per Diem and will work 8-hour, Day shifts. The RN Care Manager is responsible for carrying out effective case management functions for identified patients by planning, coordinating, negotiating, procuring and managing the care and needs of complex patients and their families throughout the continuum of care. Communicates on a daily basis with medical staff, hospital personnel and outside resources to ensure the appropriate utilization of resources according to clinical and financial standards.
