About the role

You’ll be part of Post Acute Care and Community Nursing, a service providing post-acute, subacute and community-based nursing care to patients in their homes and clinic settings. The service supports patients following hospital discharge, throughout their recovery journey, and during the chronic phase of illness with ongoing nursing intervention and chronic wound management.

• Deliver high-quality nursing care to patients in their homes and clinic settings following discharge from hospital
• Be part of a team promoting recovery, independence, and safe community living for patients
• Help reduce avoidable patient hospital presentations and readmissions
• Assess, plan, implement, and evaluate care using a reablement approach that supports recovery and maximises patient independence
• Provide wound care, clinical monitoring, health education, and community-based support within scope of practice
• Work autonomously while recognising clinical risks, escalating concerns appropriately, and maintaining patient safety

About you

• A Registered Nurse (division 1) with the Australian Health Professionals Regulation Agency (AHPRA)
• Current NSW unrestricted driver's license (class c or above) to attend clients’ homes with provided car
• Demonstrated ability to work within an interdisciplinary team using effective interpersonal skills
• Demonstrated ability to prioritise competing workloads within a complex environment
• Experience working within community healthcare settings is desirable
