Come join our team as a full-time Family Nurse Practitioner at Community Health Services- 2221 Hayes Avenue in Fremont. This is an outstanding opportunity for an experienced, compassionate FNP, who enjoys caring for a diverse population of patients--including both adults and children. Applicants must have at least 2 years of experience in a family practice setting.

$120,000 base salary, No RVU's and paid malpractice insurance. As a Federally Qualified Health Center, our providers are eligible to apply for student loan repayment through National Health Service Corps (NHSC)- up to $75,000 for full-time employees. Other benefits include paid vacation, sick time, and retirement benefits.

What You’ll Do

• ✅ Work directly with patients Monday through Friday- No Weekends
• ✅ Perform Adult and Pediatric Assessments
• ✅ Communicate clearly with Providers, Patients, and CHS Staff
• ✅ Follow-up on Lab and Diagnostic testing
• ✅ Follow all safety, HIPAA, and regulatory guidelines

What We’re Looking For

• ✅ Current License to practice in the State of Ohio as a Family Nurse Practitioner
• ✅ Current Basic Life Support Certification
• ✅ Minimum of two years as a FNP preferred
• ✅ Knowledge of documenting with EHR
• ✅ Strong communication skills and the ability to multi-task
• ✅ Ability to critically think and problem solve
• ✅ Reliable, professional, and compassionate
