Social Worker - Continuing Health Care

We are seeking an exceptional and engaging individual to join us as a Qualified Social Worker to work within our Continuing Healthcare (CHC) Adult Social Care (ASC) Team under a fixed two-year contract. This role is an interesting and exciting opportunity working within adult services to undertake Care Act assessments and reviews for those with identified complex needs with potential eligibility for CHC. You will work from a multi-disciplinary perspective and work closely with health and social work colleagues to determine NHS eligibility, ensuring legal compliance (Care Act 2014, Mental Capacity Act) and promoting independence while supporting service users and families. You must in the foremost have knowledge and working experience of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care alongside working within the duties and practice guidance of the Care Act 2014.
