Senior Practitioner - Urgent Community Response Team

We are seeking an exceptional, engaging, and committed individual to join us as a Senior Practitioner in the Urgent Community Response Team (UCRT). This is a unique and rewarding opportunity to play a key role in delivering high-quality, responsive practice within our service. UCRT is a joint team of nurses and social care practitioners and focuses on delivering timely, person-centred interventions to support individuals in the community. As a Senior Practitioner, you will take on a leadership role in practice, supporting decision-making, coordination, and oversight of complex cases. This position operates within a 7-day service, requiring flexible shift working, including weekends on a rota basis.
