CCC Program Scheduler

Responsible for facilitating the conversion of referrals to admissions through the completion of the pending management and intake processes. Using strong, effective, and collaborative communication, technology, and customer service skills, while acting as a liaison between the VITAS Sales team, Vitas Palliative Care (VPC), Care Connection Center (CCC) Intake team, referral sources, patients and families, the Admissions Field Staff and Director of Admissions. Providing education to callers on the VITAS Mission and being a strong advocate for the access and delivery of palliative and hospice services.
